The LVN states that she mistakenly saved the note as finished without completing wound care for the patient. She states that at the end of her shift she had completed and documented all of her other assessments, wound care, and medication administration for all the patients in the infirmary, as well as the other duties assigned to her by her charge nurse. She states that in her shift report to the oncoming infirmary nurse, she did report that she did not complete this patient‘s dressing change.
