On or about September 5, 2019, the LVN failed to review a patient’s allergies when the provider ordered Levaquin. Instead, she wrote a verbal order and added Levaquin to the Medication Administration Record for the patient, who had a known allergy to the drug. Her conduct was likely to harm the patient from the expected effects of an anaphylactic reaction.

Because of this, the LVN was summoned by the Texas Board of Nursing to defend her side. In response to the incident, the LVN states when she received a call for this order, she was assisting another patient. The LVN did not have the Medication Administration Record (MAR) nearby, so she took the verbal order and wrote it down. Once the LVN  was finished with the other patient, the oncoming nurse was clocked in and ready to take over the patient, so she reported the order was received, but it had not been reviewed or administered.
