False documentation is a serious case, which is why a nurse attorney is needed if you get accused of this kind of scenario. Additionally, the RN falsely documented a physical assessment on a Recertification Skilled Nursing Visit Note for a patient. Her conduct was deceptive, created inaccurate medical records, and was likely to injure the patients in that subsequent caregivers would not have accurate information to base their future care decisions.

The Texas Board of Nursing then advised her to attend a hearing regarding her issue. During the hearing., the RN states that the Administrator called all the nurses and told them that due to inclement weather, all visits performed outside of Midland should be telephonic. She states she had one recertification visit that was time-sensitive so she called the clinical supervisor and asked her what to do. She states she was instructed to complete her visits telephonically but claims she was never told that she was supposed to use a different type of visit note.

The RN states she filled out the usual OASIS and Skilled Nursing Note forms. She states she did not intend to falsify her documentation and states she used her previous knowledge of the patients for reference in documenting her assessments. The RN states she feels this situation was caused by a lack of training.
