An incident happened on or about June 5, 2018, while an RN was employed at a hospital in Conroe, Texas, and had been in that position for two (2) years and nine (9) months. The RN inappropriately diagnosed a patient with hypothyroidism, though the patient had thyroid-stimulating hormone (TSH) within normal levels. Additionally, the RN ordered unnecessary diagnostic laboratory studies, including Free T3 and Thyroid Peroxidase and Iodine Plus. The patient was then started on 60 mg of Armour Thyroid. Subsequently, the patient experienced an increase in anxiety. The RN’s conduct was likely to injure the patient from the adverse effects of unnecessary medication and supplements.
In response to the mentioned incident above, the RN states that normal lab levels for thyroid labs are reference ranges where 94% of individuals fall, so it is appropriate to treat someone on the lower end of the reference range if a patient reports symptoms. She also states that the patient reported a mild increase in anxiety after starting the new medication. The patient was told to lower the dose by half which she did not do. She adds that at the patient’s monthly follow-up visit, the patient reported feelings of increased anxiety that self-resolved without lowering the dose of medication and she reported she was tolerating the medication well.
As a result, the RN’s conduct put her into disciplinary actions by the Texas Board of Nursing. She failed to hire Texas BON lawyers to fully defend her case and have led to this decision by the Texas Board of Nursing.
