At the time of the initial incident, an RN was employed as a Registered Nurse at a medical facility in Harlingen, Texas, and had been in that position for one (1) year and three (3) months. 

On or about May 12, 2018, while employed as a Registered Nurse, the said RN failed to properly administer Heparin to a patient in that the RN injected a subcutaneous tuberculin syringe into the patient’s intravenous needleless system. The conduct of the RN may have degraded the needleless hub, which had to be replaced, exposing the patient to the risk of infection. 

It was on or about June 17, 2018, while employed as a Registered Nurse, the RN failed to ensure safety measures, including a bed alarm, were activated for the patient, a documented high-fall risk patient; subsequently, the patient experienced a fall resulting in fractures of the hip socket and pelvis. The RN’s conduct unnecessarily exposed the patient to harm and may have contributed to the fall. 

Concerning the events that happened, according to the RN, the ordered Heparin dose was too small to accurately measure with a 3ml syringe, so she immediately replaced the hub with a new one after injecting the tuberculin syringe containing the Heparin into the needleless port system. The RN mentioned that the nursing assistant turned off the bed alarm while they were assisting the patient and she was unaware the bed alarm had not been turned back on when the RN departed the room.

Because of this incident, the Texas Board of Nursing then subjected the RN and her license to disciplinary action.
