On or about May 20, 2020, while employed as an RN at a hospital in Grand Prairie, Texas, RN failed to accurately and completely document the pain assessment for a patient in that RN failed to document the location of the patient’s pain. In addition, RN documented the administration of scheduled Tylenol for the aforementioned patient at 10:00 and 16:00 when there is no record of RN removing Tylenol from the medication dispensing system for the patient. RN also failed to accurately and completely document the dose of the Tylenol administered. RN’s conduct resulted in an incomplete medical record and was likely to injure the patient in that subsequent caregivers did not have accurate and complete information on which to base their decisions for further care.
In response to the above incident, RN states that he rounded on all of his patients that day and asked if any of his patients were in severe pain. RN states that none of his patients reported pain at the time that he rounded. RN states that all assessments were recorded and charted.
The above action constitutes grounds for disciplinary action in accordance with Section 301.452(b)(10)&(13), Texas Occupations Code, and is a violation of 22 TEX. ADMIN. CODE §217.11(1)(A),(1)(B),(1)(C),(1)(D),(1)(M),(1)(P)&(3)(A) and 22 TEX. ADMIN. CODE §217.12(1)(A),(1)(B)&(4).
The Texas Board of Nursing gave the RN enough time to defend the complaints filed against him.
