On or about February 2, 2009, while employed in a Christian hospital in Texas, the LVN failed to administer Androgel to a patient as   ordered by the   physician.  Additionally, she   erroneouslydocumented that she administered said medication in the patient’s medicationadministration record (MAR). Her conduct created an inaccurate medical recordand may have injured the patient in that failing to administer medications asordered by the physician could result in non-efficacious treatment.

On or about February 12, 2009, while employed in the same medical facility in Texas, the LVN failed to administer scheduled 9:00 medications to a patient, who was transferred from ICU to a telemetry unit. Her conduct may have injured the patient in that failing to administer medications as ordered by the physician could result in non-efficacious treatment.

In response to the accusations filed against her, she states that she was asked by the assistant manager if she would take a triple assignment so that another LVN could take an admission. Shestates that the LVN gave her a truncated report at around 10:30 and told herthat everything had been done for the patient and all she had to do was”babysit” until the patient got transferred.

She states that 30 minutes after the patient was transferred, the accepting nurse called and stated that thepatient’s 9:00 medications had not been administered. She acknowledges that itwas her responsibility to make sure that everything had been done when shereceived the patient.

The result of her actions left the medications unaccounted for. The Texas Board of Nursing noticed this andadvised the LVN to defend her against the case. She did not seek a nurseattorney’s assistance.

Therefore, the Texas Board of Nursing placed her LVN license under disciplinary proceedings to avoid further incidents regarding wastage procedures.
