On or about November 3, 2016, while employed as a staff nurse in a memorial healthcare center inHumble, the LVN failed to escalate nursing interventions in that she did not notify the Rapid Response Team(RRT) and physician when a patient had a change in condition, including a blood
Instead, she increased the infusion rate of the patient’s IV fluid without a direct physician’s order. In addition, the LVN failed to provide adequate information on the patient’s condition to other nursing staff and the RRT.
The LVN’s conduct could have deprived the physician and caregivers of vital information which would be required to institute timely medical interventions, thereby potentially placing the patient at risk of complications of hypotension, including loss of consciousness, coma, or death.
Because of this, theTexas Board of Nursing summoned the LVN to defend herself. She states that she instructed the CNA to chart the blood pressure reading from the arm without the N, and that
In addition, the LVN states that when the night nurse made rounds at 7 pm, the night nurse accepted responsibility for the patient. She asserts that she was not involved in the rapid response because the night nurse called the rapid response at 7:30 pm and she states her shift ended at 7 pm.
The following incident and defense against the case caused the Texas Board ofNursing to place the LVN and her license into disciplinary proceedings.
