On or about September 11, 2019, through January 2, 2020, while employed as a Licensed Vocational Nurse and while assigned to provide nursing care for the Patient, the said LVN completed and submitted incorrect nursing visit notes for the Patient including incorrect start and end times on her nursing visit notes for Patient. The visit notes also incorrectly included the caregiver’s signature on the LVN’s visit notes. The LVN’s conduct was wrong, created an inaccurate medical record, and could have injured and confused the patient and facility in that subsequent caregivers might not have accurate information to base their future care decisions and may have affected payroll decisions, respectively.

In light of the occurrence, the said LVN acknowledges her mistakes concerning inaccurate nursing notes for visits with incorrect start and end times, and she admits that she made a mistake and attributes her conduct to an isolated lapse in judgment.

However, the Texas Board of Nursing found that his conduct violates the Texas Occupations Code. As a result, he receives disciplinary and further sanctions from the Texas Board of Nursing (BON).
