On or about August 18, 2017, the LVN also submitted two different skilled nursing notes for the same date for a patient. This conduct resulted in a conflicting medical record and was likely to injure the patient in that subsequent caregivers would not have complete information on which to base their care decisions.

TheTexas Board of Nursing then advised her to attend a hearing regarding her issue. She admitted to documentation issues. She advised her charting will improve. Therefore, the Board decided to extend her disciplinary proceedings for several more years to ensure that she will perform even better in the future.
